Beech Street Corporation, the nation’s leading independently owned Preferred Provider Organization (PPO), has introduced a new comprehensive Disease Management program.
Through a partnership with Landacorp, a SHPS company, Beech Street’s total health and wellness management program assesses general health and manages chronic health conditions such as asthma, diabetes, and heart disease. Its goal is to engage the patient population that is not reached by traditional Utilization Management programs.
The Beech Street program provides personalized information and behavior modification tools customized to help individuals take control of particular conditions and live healthier lifestyles. Participants are furnished with materials including surveys, personalized feedback, home testing kits, provider materials, and seasonal newsletters.
A distinct advantage of the program is Beech Street’s nurse intervention practice for those patients at high risk due to chronic conditions.
