In the State of Maryland the rates charged for many medical procedures are set by the Health Services Cost Review Commission ("HSCRC"). If the rate for the medical procedure is regulated by the HSCRC then Beech Street rates will be equivalent to the state set rates. Beech Street negotiated rates may be available for services not subject to state regulation.

TEXAS REQUIRED NOTICE: A Texas facility marked with a ¹ indicates that the facility may have a facility-based physician or facility-based health care practitioner, such as an anesthesiologist, radiologist, pathologist, neonatologist or emergency department physician, that is not included in the network. Non-participating facility-based physicians and non-participating facility-based health care practitioners may balance bill the enrollee for amounts not paid by the enrollee's health benefit plan.
