FARA and Total Benefit Services have now returned to their offices in Metairie. All of their regular phone and fax numbers are operating.
Any claims mailed to the temporary Houston address will be forwarded for 6 months, as of November 17. Claims should now be mailed as follows:

Our thoughts and sympathies go out to the thousands of people
who have been impacted by hurricanes Katrina and Rita. It is our
sincere wish that you and your families have remained safe.
In order to ensure that evacuees are not turned away in this time
of need, we are reaching out to those in our provider community who
may be experiencing an influx of patients seeking care outside the
state of Louisiana.
In addition to caring for those clearly identified as Beech Street
patients, we are asking that you also accept patients associated
with BestCare, Inc. of Baton Rouge, Louisiana. Many individuals’ ID
cards do not yet carry the Beech Street logo, as the transition
since the acquisition is still in progress.
If any BestCare individuals seek care with you,
please treat them as if they are Beech Street patients. You can
identify them by locating the BestCare logo on their ID
cards:
Our client GreenTree Administrators was impacted by
hurricane Rita. They can be reached at
800.825.2117. Their Utilization and Case
Management departments are operating, but there may be delays in
service.
